Toxina botulínica tipo A en la prevención de la cicatriz hipertrófica: del mecanismo molecular (TGF-B1/CTGF) a la optimización de dosis, momento y técnica. Revisión narrativa de la literatura

ntroducción: la toxina botulínica tipo A (TBA) se propone para prevenir la cicatriz hipertrófica, pero la relación entre su mecanismo y la pauta óptima sigue sin definirse. Objetivo: revisar de forma narrativa el vínculo mecanismo–dosis/momento/técnica. Métodos: búsqueda en cinco bases (2015–2025, inglés), con estudios clínicos y fisiológicos incluidos. Resultados: la TBA temprana mejora la calidad de la cicatriz frente al control en distintas localizaciones y reduce TGF-B1, CTGF, proliferación del fibroblasto y colágeno; la dosis óptima depende del sitio. Conclusión: complemento prometedor en pacientes de alto riesgo, sin esquema estandarizado; la evidencia sobre técnica es insuficiente.
