Determinantes sociales y calidad de vida en el cáncer de mama: evidencia desde una población mexicana

Introducción: El cáncer es un problema de salud pública al ser la segunda causa de mortalidad mundial. En México la incidencia fue de 195,499 en 2020, siendo el cáncer de mama (CM) el que más afecta a las mexicanas. Objetivo: Analizar los determinantes sociales que inciden mayormente en la calidad de vida (CV) de pacientes con CM atendidas en un hospital mexicano durante 2023. Material y métodos: Estudio transversal analítico, con pacientes de 18 años en adelante con diagnóstico de CM en los últimos 36 meses en estadio I-III, que recibían atención médica y quimioterapia en el Hospital General León. Se emplearon tres instrumentos: 1) Cuestionario de CV (QLQ-30, siglas en inglés), 2) su complemento para CM (QLQ-BR-23), ambos elaborados por la European Organisation for Research and Treatment of Cancer y, 3) cuestionario elaborado para determinantes sociales. Resultados: 73 participantes. Puntaje global de CV 77.62. La quimioterapia neoadyuvante causó más dolor (p=0.021). Tener compañero se asoció a mejor funcionamiento sexual (p=0.023), no tener vivienda propia incrementó sintomatología mamaria (p=0.030), a menor ingreso familiar mayor náusea y vómito, mejor imagen corporal y peor funcionamiento sexual (p=0.020, 0.053, 0.007, respectivamente). Asociación positiva de CV con funcionamiento físico, emocional y social; y preocupación por futuro (p=0.015, 0.006, 0.005 y 0.037 respectivamente), negativamente: fatiga, dolor, imagen corporal y efectos secundarios al tratamiento (p=0.004, 0.003, 0.023 y 0.006 respectivamente). Conclusiones: Se encontró que los DSS que influyeron de forma significativa con la CV de las participantes fueron contar con compañeros, no contar con vivienda propia y menor ingreso familiar. Los factores que tuvieron una asociación negativa con la CV fueron la fatiga y el dolor, los ítems de imagen corporal, efectos secundarios de la terapia sistémica y preocupación por el futuro.
